CBTI doesn’t fail in theory. It fails in structure.

You know the model. You understand stimulus control and sleep hygiene. You’ve reviewed the research.

But implementation is different than training.

Sessions feel inconsistent. Materials are scattered across folders. You explain CBTI slightly different every time. Your website description is either too technical or too vague. Referrals aren’t coming in steadily. You’re delivering solid treatment, but there’s no infrastructure around it, no standardized packet, no defined session progression, no outcome tracking, no clear positioning.

That’s common. And fixable. Specialties become strong when they are designed, not improvised.

I built my CBTI program intentionally starting in 2021, mapping session objectives, developing structured psychoeducation materials, integrating ISI and PSQI tracking, and aligning referral language around a focused sleep practice. I’ve pivoted along the way learning from the patients and referring physician feedback.

I now help other therapists build that same internal structure so their specialty feels deliberate rather than pieced together.
